Panel concludes BP missed warning signs

A panel investigating the oil rig explosion in the Gulf concluded that BP ignored or missed warning signs.

BP's failure to learn from previous problems and insufficient worker and response training are both cited as reasons for the April blowout.

The explosion killed eleven workers and caused the largest offshore oil spill in U.S. history.
